Insurance card — We'll verify your coverage and explain your benefits before treatment.
Photo ID — A driver's license or state ID is helpful for our records.
List of current medications — Especially important for patients managing chronic conditions.
Dental records or X-rays — If transferring from another dentist, any recent records are helpful (not required).
Arrive 10 minutes early — Gives us time to get everything set up so your appointment starts on time.
